IMO, the person calling the name should not use those, unless they know for certain that it will be received with appreciation (not just tolerance, or worse). The sex of the caller doesn't change the fact that it might be an insult. And then, there is the difference between calling someone a name to their face and using it about them to a third party. LisaH hasn't chimed in, so all this is just a general quideline, not putting words in her mouth.
