How are your punctuation skills?

So, there are these two guys writing an article to a magazine or whatever. Their editor had given them some guide lines when to use this form and when some other form. "had had" -form was to be used in some exact places. For some reason the other guy (Jones) did not use "had had" -form when he was supposed to (since the editor had approved it), but used "had" -form instead... :S

Does it make any sense now?
