Well, it depends upon the type of revolver, doesn't it? Some old ones don't have the cylinder rotate outward for unloading. Instead, you have a rod which rotates over in front of a chamber, and you have to open a loading gate, rotate the cylinder by hand, and punch out the cartridges one at a time with the rod. That requires a lot of manipulation.

None. But for the type like I mentioned above, there's a lot going on. You cradle the gun in your left hand, while rotating the cylinder with several fingers of that same hand, trying to get the chambers to line up with that push rod, and with the right hand you're pushing that plunger to eject the cartridges, all while holding the gun pointed upward so that the cases fall out. This is an older type revolver, granted, but they're not as easy as the new-fangled ones. And with all that finger action, I could see how someone could accidentally get one caught inside the trigger guard. Not making excuses, of course, it shouldn't happen, but just trying to figure out how it happend. I'd like to know what kind of revolver it was.
