Different malfunction possibilities, severity, and statistics.

I am sure this has been done at some point, and perhaps the community can just point me to another post, poll, or link.

I am just wondering what all the different malfunction possibilities were when deploying main, and or reserve, what the severity of those malfunctions were, and there likelihood.

I am sure user experiences vary greatly. I am sure there is the beginner who had a malfunction early in their jumping career, and veterans that have experienced several malfunctions, and maybe that rare soul that has logged thousands of jumps and never experienced any malfunction.
