"Doctor" is a title going back to 1213 and is a license to teach, originally authorized by the pope. The PhD dates back to 1652. Sometime in the 19th Century medical people (mostly barbers and quacks in the USA in those days) did a pretty good job of appropriating the title to describe their occupation. In, for example, the UK, the more advanced physicians and surgeons (FRCS, FRCP) generally return to the old ways and do not call themselves "doctor" unless they actually have a genuine research degree. Preferably for life. A qualified physiotherapist gives you a treatment plan designed to help you move along.
