If you have a 2007 SIM, the address is outdated. 5401 Southpoint Centre Blvd., Fredericksburg, VA, 22407 (540) 604-9740 fax (540) 604-9741 After you finish filling out your yellow card, take a minute to look the address or fax number online to make sure your sending it to the right place.
