hard opening leads to two out

im just wondering if this has ever happened to anyone before. one of our jumpers went to pull, missed and as he was reaching for a 2nd time got into a roll and slightly head down. pulled and the pilot chute went under his arm and across his chest. the report was that "he pulled and bamm he had a fuckin chute". opening was so hard that it knocked his shoes off and also ripped his altimiter off of his hand(most likely due to the lines running down his arm). not only that but the stiching that held his risers in ripped partially. the risers raised enough to catch his rsl which pulled his reserve. it pulled so hard that there is an almost 90 degree kink in his rsl. he landed fine under the two canopies but probably tore some ligaments in his shoulder(waiting on the doc's report). fucked up huh? just wondering if that has been seen before. and hope to get you back soon scottie 2 hottie

Odd!
I have never heard of a kinked RSL.
However, I have seen plenty of reserve ripcords that were premanently kinked after RSLs pulled them.

What type of container was buddy jumping?

If you S-fold the RSL under the top of the reserve pin cover, it can get a kink in it.

What do you mean by this? Do you mean one of the riser covers? And if so, where - on top of the shoulder on the RSL side?

Does anyone have an idea how the RSL got "caught" by the risers? The RSL is going to be connected to one riser on one end (if it isn't a Racer), so it's normal for that end of the RSL to get moved around a bit when the main risers come off the jumper's back and stick up into the sky. RSLs are supposed to be long enough, I think, that this movement won't try to pull/pull on the reserve ripcord. So what happened to this guy that changed that? Did his body swing so far forward under the canopy during the hard opening that the RSL connection point on the riser was at the maximum possible distance from his nape (where the RSL goes around the bend to get to the reserve ripcord) and the RSL length became insufficient?

And if it's something like that, was that RSL the correct one for that rig?

I think I'm going to ground-test my rig tomorrow to see if the RSL will pull the reserve in the event I rotate 180 degrees backwards (i.e. feet forward, head back) in my 3-rings.

It ripped where the reserve risers were stitched to the reserve pack tray. I can't remember what other damage there was but the container had to be sent to the manufacturer to be repaired.

When the stitching starts getting ripped away, if it is where I think it is, it changes some of the geometry and that can lead to the RSL pulling the reserve. I have seen this happen on a couple of rigs with the person dumping as described.
