The team Captain is to fill out the Team registration form. It would be easiest if one person would fill out all the forms and fax them back. The fax number is located on the credit card authorization form. If individuals wish to do this on their own, please put your team name on ALL of your forms so that we may credit the proper accounts. * Please note the credit card authorization form must have a signature in order to process.
