The reminder has been issued following a close call in which a woman nearly came out of a poorly adjusted harness that was left very loose in an effort to make her more comfortable. The student harness was equipped with a Y-strap, which apparently played an important role in keeping her in the harness after the main canopy was deployed. This notice is actually a terrific reminder to every tandem instructor, regardless of the type of tandem equipment used for the jump.
