Incidents Form

I wonder what your thoughts would be about creating a form to filled out prior to posting an "Incident." There has been more than one time that it would appear that there is a rush to be the first to post rather than provide quality of information.

Many times, people have to ask the same questions, W/L, Main canopy, Reserve type, AAD, RSL, etc.

If there were drop down boxes for injury, fatality, gear, and an open text box for additional information, etc.

Now that is a really good idea! Standardize the format and such, make things cleaner and more efficient.

After reading the 2/16/2005 Skydive Arizona fatality report I had this same thought. Most all of the relevant information was presented in an easy to read format. A form to standardize would also make it easier to search and coorelate incidents over time.

There is a form for fatalities that links into the Database. This provides a lot of search fields as it becomes populated with current and past info.

As for other incidents that are non-Fatal a form would be nice.
